To understand how a facelift works and why the face ages in a certain way, we need to understand the internal anatomy of the face.

The face is not just skin – it is a complex structure in which each layer plays a role in shaping our appearance. As we age, changes occur in the skin, fat pads, muscles, ligaments and bones. This process is not simple, as all of these layers interact with each other, and the loss of support structures in one layer affects all of the others.

What are facial ligaments and why are they important?

Ligaments are strong, elastic bands of connective tissue that connect and attach skin and muscles to deeper structures, primarily bones and fascia (the connective sheath of muscles).

We can think of them as anchors or ropes that hold the skin and subcutaneous tissue in place, allowing the face to maintain its youthful tension and shape. When we are young, ligaments are strong and elastic, keeping all the layers of skin, fat, and muscle properly aligned.

However, over time, the ligaments loosen, which causes:

🔸 Soft tissue sagging – The cheekbones and cheeks lose their full, lifted appearance and begin to droop downward.

🔸 Creation of wrinkles and folds – As the tissue descends, the skin "folds", leading to more pronounced nasolabial folds and marionette lines.

🔸 Loss of defined jawline – Fat tissue that was located higher on the cheeks moves downwards, creating, for example, the so-called jowls (hanging skin on the jaw).

What is the SMAS layer and why is it crucial in a facelift?

SMAS (Superficial Musculoaponeurotic System) is a layer of connective and muscle tissue that is located under the skin and subcutaneous fat, and above the deeper muscles of the face. This layer plays a key role in the structural support of the face, as it connects the muscles to the skin and allows them to work together for facial expressions and movements.

As we age, the SMAS layer stretches and relaxes along with the ligaments, leading to sagging soft tissues and the formation of wrinkles. This is one of the main reasons why skin tightening alone cannot provide long-lasting results – because it is the SMAS layer that actually holds the skin in place.

How does a facelift help?

A facelift not only tightens the skin, but also the deeper layers of the face, including the SMAS layer, which is connected to ligaments. This is important because the skin is a structure that naturally stretches very quickly. If we were to tighten only the skin, the underlying structures would quickly stretch it again and all the wrinkles would return. During the procedure:

Sometimes we recommend a combination with other treatments, such as blepharoplasty (eyelid surgery) or lipofilling (transfer of your own fat to the face to improve volume).

2. Preparation for the procedure

The patient receives detailed instructions on preparation, including avoiding certain medications that may increase the risk of bleeding (such as aspirin) and quitting smoking, as it can slow healing.

3. Anesthesia and the beginning of the operation

A facelift is most often performed under general anesthesia or deep sedation. The incisions are carefully placed – usually along the hairline and around the ear, so that they are as invisible as possible after healing.

4. Tightening of deeper structures and removal of excess skin

A key part of a facelift is working on the SMAS layer, which is lifted and repositioned to achieve a long-lasting and natural result. Just removing the excess skin without treating the SMAS would not give lasting results, as the skin would quickly relax again.

5. Closing the incisions and the final phase

Once the tissues are precisely positioned, excess skin is removed and the incisions are closed with fine stitches. If necessary, small drains are placed to reduce fluid buildup and swelling.

6. Recovery and results

The patient remains under observation for several hours or, if necessary, overnight. In the first few days, there may be swelling, bruising and mild discomfort, but painkillers help to reduce the discomfort.

The most pronounced swelling subsides within two to three weeks, while full results are seen after a few months. The scars are discreetly placed and become barely visible over time.
